*A trainee is defined as an individual who is officially in training, such as a resident physician, or someone in a fellowship, graduate program, or post-doctoral program. Trainees should submit a letter verifying their educational status to firstname.lastname@example.org or fax to 312-202-5003 in order to receive the discounted registration rate.
The Early Registration Deadline is Monday, April 15, 2019. Online or faxed registrations should be received by midnight CT on Monday, April 15 to ensure the early rate. Mailed registration forms should be postmarked by the deadline date. All registrations received after the deadline will be charged the late registration fee.
A $75 discount is available for all Active ISOM Members with dues paid in 2019. If you would like to become a member or update your membership status in order to receive the discounted rate, please visit http://www.otitismediasociety.org/membership.html. If you recently paid your dues, please allow 1-2 business days for your membership to be updated before completing your online registration.
Confirmations will be sent within 10 working days. If you would like to receive your confirmation via e-mail, please provide a valid e-mail address with your registration. If you require a guarantee of receipt, send registration forms by an overnight carrier who can provide you with confirmation of delivery.
Substitution and Cancellation Policy
Substitutions: Substitution of one individual for another is not allowed.
Cancellations: Requests for refunds must be made in writing and received by ISOM Registration Services on or before Friday, May 17, 2019. There is a $75 handling fee for ALL refunds and returned checks. Cancellations and registrations postmarked after the deadline date will not be eligible for refunds. Substitutions from one registrant to another are not permitted.

ISOM Registration Services
The Scientific Program Committee will be selecting awardees for 14 scholarships of $800 for domestic travel and 1 scholarship of $1000 to support travel from overseas to attend the 2019 Symposium. Award winners will be selected from among the Call for Abstracts submissions and/or by Invitation (see below). Winners will be announced at the Opening Session of the Symposium.
- We will award 9 US trainees a travel scholarship of $800.00 each, (competitively selected from abstract submissions)
- We will award 5 US Minority travel scholarships of $800.00 each (selected from submitted abstracts and/or invited speakers)
- We will award 1 Foreign underrepresented minority (URM) trainee at $1000.00 (competitively selected from abstract submissions)
The representation of certain groups of people in science and engineering (S&E) education and employment differs from their representation in the U.S. population. Women, persons with disabilities, and three racial and ethnic groups – blacks, Hispanics, and American Indians or Alaska Natives – are underrepresented in S&E. Individuals identifying as members of these groups are encouraged to submit an abstract to be considered for scholarships to attend the 2019 Symposium.
